School of Nursing

Room Key Policy

Room keys in the School of Nursing are the property of the University of Washington.  They are issued to individual students, faculty, staff, and selected visitors with the understanding that they not be released to other persons, and that they are not duplicated.  The key holder remains fully responsible for loss or damage to any key while it is assigned to him/her.  All key must be returned to the School of Nursing upon temporary separation or permanent termination from the school or upon request.  The failure to return a key constitutes misappropriation of university property and will be handled accordingly.  A Key Agreement reflecting the above understanding must be signed by the recipient for each key issued to him/her.

Eligibility to Hold Keys

Keys are issued to School of Nursing employees and students for rooms to which they require unrestricted entry.  Keys are issued to others (vendors, visiting scholars, and others) on an as-needed basis.  Only one key per room will be issued to any given person, and that person will be held fully responsible for that key.  In all cases the issuing of keys requires the approval of the dean, an associate dean, a department chair, the school’s administrator, or a designated key coordinator(s).  Unit administrators determine criteria for approval.

Lost Keys

Lost keys must be reported to the unit administrator or key coordinator as soon as possible even if a replacement key is not requested.  Lost keys will be reported to the Health Sciences Security Office which will determineif a replacement key can be issued or if locks must be changed.  If it can be documented that the lost key cannot possibly be retrieved and/or identified with the University of Washington, a replacement key will be issued.  If there is the possibility that the key could be found and used, all associated locks must be changed and new keys issued to everyone involved.  A portion of these costs may be passed on to the keyholder depending on the circumstances (see Fees for Replacement Keys below).

Fees for Replacement Keys

A first key will be issued at no charge beyond any required deposit.

A second key to replace a lost or damaged key will be issued at a minimum charge of the cost of recutting a new key.  The Health Sciences Security Office will review each case and notify the school of actions required(e.g. necessary lock changes) and advise on associated additional costs that my be passed on the key holder.

A third key for the same room will be issued only following a review and recommendation by the Health Sciences Security Office, and the approval of the unit administrator and/or Dean.  Charges imposed will be determined on a case-by-case basis taking into consideration any negligence or policy violations involved and actual costs incurred in changing locks and issuing new keys.

Keys not returned within a reasonable time of termination/separation or upon request will be considered lost, and appropriate replacement fees will be assessed on the key holder of record.
